I messengered a friend of my family in Italy who is a grandfather and asked him if this custom in Italy is still observed on January 6th. Indeed he said it did and his grandchildren all had goodies in their stockings yesterday in Livorno, Italy (where St. Elizabeth Ann Seton converted, btw).
He said bad children to get coal, but it is something that looks like coal (chocolate) and is delightful to eat!
