There is a hole the size of a half dollar in the carpet of the sanctuary at Our Lady of Lourdes that I own. I wasn't trying to swing the thurible in a circle, but I did bump it on the edge of the altar and the charcoal fell out!
Our thurifers swing the thurible in a full circle during the processional and recessional on major feast days.
At the ordination of a friend of mine, during the incensation by one of the concelebrating deacons, the incense came out of the censer and fell right in front of the royal doors. Burned a hole in the new carpet of the cathedral. The bishop was present of course.
Swinging the thurible in a full circle is an Anglican affectation, although it does happen in the Ambrosian rite, despite the fact that the thurible has no cover.
